Aetna Global Benefits — World Traveler

AGB is a medical indemnity program for employees and accompanying dependents traveling outside their home country for business trips.

Dental Coverage

Callidus offers a PPO dental plan through Aetna Insurance. It offers nationwide access to thousands of Aetna providers. To access nationwide Aetna Dental PPO providers, please visit www.aetna.com

Vision Coverage

Vision coverage is provided by Vision Service Plan (VSP). Benefits are paid in full up to plan allowance; the plan pays for one eye exam every 12 months.

Life Insurance Programs

• Basic Life & AD&D Insurance
  Life Insurance and Accidental Death and Dismemberment (AD&D) are provided and paid for by Callidus Software. Coverage is two times your base salary for both basic life and AD&D.
• Business Travel Accident Insurance
  An additional Life & AD&D Insurance that covers an employee while traveling on business. The policy pays up to two times an employees’ annual salary to a maximum benefit of $500,000. The BTA Insurance is provided by Cigna Life Insurance.
• Supplemental Life Insurance
  You may purchase additional insurance for yourself, your spouse / domestic partner, and your dependents. This program is offered through Aetna Life Insurance.
• Supplemental AD&D Insurance
  Additional AD&D Insurance can be purchased to cover either yourself only or yourself and your entire family.

Short-Term Disability

In the event of a short-term disability, employees receive 60% of their base income up to a maximum of $2,309 a week. Employees are eligible for short-term disability immediately.

Long-Term Disability

Following 90 days of continuous disability, employees are eligible for long-term disability coverage, and will receive 60% of your base income up to $10,000 a month.

Employee Assistant Program (EAP)

Callidus Software recognizes a need to provide a resource to help employees balance life and work. EAP is a free confidential counseling and referral service, which is designed to help employees and their household members. The provider is CIGNA Behavioral Health.

Flexible Spending Accounts (FSAs)

Flexible Spending Account allows employee to set aside money on a pre-tax basis to pay for medical and dependent care expenses. It is a voluntary plan and regulated by IRS.

Compensation

401(k) Plan

Callidus Software offers a 401(k) Plan administered by Wachovia. The Plan allows employees to contribute on a pre-tax basis up to 80% of eligible compensation to an annual limit of $16,500. Callidus Software also offers a pre-tax catch up provision of $5,500 for participants over the age of 50.

ESPP Program

The Callidus Software ESPP makes it easy and financially attractive to participate in ownership of the company that employees are helping to build. The ESPP program offers semi-annual open enrollment periods. Employees at all levels are eligible for the Callidus Option Plan. Under the plan, employees have the option to purchase Callidus stock at a set price, subject to the Board of Directors' approval. Employees' right to purchase shares under this plan is subject to a vesting period.

Vacation Pay

Full-time employees accrue vacation hours on a per pay period basis. Employees on staff for one month to 36 months receive 15 days or 120 hours per year (180 hours maximum accrual). Employees on staff for 37-59 months receive 15 days or 120 hours per year (200 hours maximum accrual). Employees on staff for 60 months and over receive 20 days or 160 hours per year (240 hours maximum accrual).

Sick Pay

Full-time employees receive 5 days of sick time annually. New hires receive sick days on a prorated basis.

Callidus Software observes 11 holidays per year, including:

• Day After New Years Day, January 2
• President's Day, February 16
• Memorial Day, May 25
• Independence Day, July 3
• Labor Day, September 7
• Thanksgiving Day, November 26
• Day after Thanksgiving, November 27
• Winter Holidays — December 25 and December 31
• One Floating Holiday, which must be taken within the calendar year
